GeneralPosition Summary


Under general supervision, assists leadership in supporting and managing the advanced practice provider (APP) group within a specific service line.Responsible for diagnosing and treating patients in collaboration with physician colleagues.Provide continuity of patient care by facilitating and coordinating communication between the health care team, patient and patients family.Coordinates care to ensure quality, cost effective care; appropriate length of stay; and patient safety.Serves as link integrating relevant research and best practices.

II.Principal Responsibilities


The following statements are designed to describe the general nature and level of work being performed by the provider assigned to this classification.This description is not intended to be an exhaustive list of all job responsibilities performed by this individual.

1.Clinical Leadership/Outcomes

A.Demonstrates mastery in clinical assessment, treatment and decision making including diagnostic reasoning, problem solving.

B.Serves as a mentor to assist other APPs to develop expertise in clinical skills, decision making, diagnostic reasoning and problem solving.

C.Mentors others in leading development and implementation of evidenced based best practice.

D.Serves as interdisciplinary team leader or committee chair who canlead initiatives to improve patient outcomes inside UMMC as well as outside of the organization.

E.Takes the lead in difficult situational decision making, viewed as a resource forother healthcare team members.

F.Leads or mentors others in leading quality improvement initiatives in area of practice.

G.Serves as a role model educator for nurses and other healthcare personnel.

2.Clinical Responsibilities

A.In collaboration with the attending physician, provides day to day organizational and clinical direction and leadership of the healthcare provider team.

B.Performs comprehensive history and physical assessment for patients admitted/scheduled to service/practice setting.

C.Orders, obtains and interprets appropriate diagnostic tests.

D.Establishes medical diagnosis based on history, assessment, and diagnostic findings.

E.Designs, orders and documents appropriate treatment plans/plans of care including prescriptions of medications based on a comprehensive review of HPI and diagnostic results.

F.Initiates protocols/clinical pathways, evaluates the plan of care ensuring timely intervention.

G.Performs invasive procedures.

H.Collaborates with attending physiciansand trainees regarding admission, discharge and transfer decisions.

I.Presents patients in the APP caseload on rounds.

J.Communicates plan of care with appropriate providers including the physician, the primary care provider, the referring provider, the nursing staff, and the case manager and the interprofessional team

K.Communicates plan of care with the patients and family members.Provides patient and family instruction related to the plan of care, disease process, new treatment plans, and medication regimens.

L.Counsels and educates patients and family members in disease process, health maintenance and health promotion activities.Provides primary, secondary, and tertiary preventive care services.

M.Initiates appropriate referrals and specialty consultations or other agency involvement as needed.

N.Facilitates patient flow. Collaborates with the multidisciplinary team including case management ensuring appropriate and timely discharge.

O.Provides specialty consultation.

P.Advocates for patients ensuring patient/family participation in care, knowledge of treatment options, and understanding of patient rights.

Q.Considers the holistic needs of patients and orchestrates resources (i.e. Pain service, Palliative care)

3.Education Responsibilities

A.Mentors other APP to precept novice APP or students

B.Holds a clinical or adjunct faculty appointment

C.Presents course lectures at an accredited institution

D.Provides education at multiple local, national or international meetings/conferences

E.Mentors others in presenting at conferences

F.Assists other APPs with developing in-service education for staff development

G.Nationally recognized as an expert in the field

4.Participates in the development of four of the following:

A.DNP/PhD Enrollment

B.Mentors others in presenting at conferences

C.Mentors another to lead community activities

D.Serves on a dissertation committee

E.Mentors others to publish in a peer-reviewed journal or book chapter

F.Serves as peer reviewer for professional publication

G.Publishes peer-reviewed article in professional journal or authors a book chapter

H.Edits or assists with publication efforts

5.Professional Development and Organizational Leadership

A.Serves on department, hospital and community committees.

B.Participates in and presents at local, regional, and national professional organizations.

C.Promotes the organization to all customers.Acts as a loyal and supportive informed spokesperson for the hospital.

6.Research Responsibilities

A.Serves as a clinical trial sub investigator participating in or coordinating all phases of industry sponsored research.

B.Doctorally prepared nurse practitioners are expected to conduct quality improvement ornursing research as the principle investigator adding to the body of knowledge in the nursing domain.

III.Education and Experience

1.Master of Science Degree with completion of aNurse Practitioner program.Registered and currently recorded with the Maryland Board of Examiners of Nurses and Certified as a Nurse Practitioner in the State of Maryland.Certified in area of specialty.

2.DEA and CDS eligible

3.ACLS/BCLS as required

4.Seven years(7) experience as a Nurse Practitioner is required.

IV.Knowledge, Skills and Abilities

5.Demonstrated ability providing leadership and supervision.

6.Demonstrated ability to promote professionalism through involvement in professional organizations, teaching, research, publishing, and/or certification in area of specialty.

7.Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served.Ability to demonstrate knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.

8.Highly effective verbal and written communication skills are required to interact with patient families, departmental units, medical and nursing staff on all essential matters.Demonstrated/documented effective interpersonal skills.

IV.Working Conditions

1.Weekend, shift work, holiday, on-call and overtime may be required.

2.There is potential for regular exposure to patients possessing infectious diseases in the work environment; requires observance of "Universal Precautions" by wearing gloves, goggles, and mask, is required.

VI.Patient Safety

Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.

1.Takes action to correct observed risks to patient safety

2.Reports adverse events and near misses to appropriate management authority.

Identifies possible risks in processes, procedures, devices and communicates the same to those in charg
