Cuyahoga County, Cleveland, has roughly 375,000 Black residents (~30%). Cleveland's east side has among the country's widest neighbourhood-level infant mortality gaps; First Year Cleveland, a county-hosted collaborative between MetroHealth, University Hospitals, and the Cleveland Clinic, has reduced the county's Black infant mortality rate by about a third since 2015.
