South Dakota voters adopted expansion through Constitutional Amendment D in November 2022 with 56% of the vote. Coverage began July 2023. Dakota Rural Action and Great Plains Tribal Leaders' Health Board ran the ballot campaign's rural and tribal outreach.

The number understates true eligibility: every state has Black eligibles who aren't currently enrolled, usually because of the administrative-churn rates that rose sharply during the 2023 unwinding.
