Who qualifies and how the income limits work
Washington Apple Health uses these income limits: Pregnant people up to 215% of the poverty level (about $58,740 a year for a family of three in 2026); Children, through Medicaid or CHIP, up to 317% of the poverty level (about $86,600 a year for a family of three in 2026); Adults 19 to 64, including parents, up to 138% of the poverty level (about $37,700 a year for a family of three in 2026).
These limits include the 5% income disregard federal rules apply (42 CFR 435.603). The state makes the final decision based on your household and income.
Washington has expanded Medicaid, so adults 19 to 64 with income up to 138% of the poverty level can qualify.
For Black families
Apple Health has paid certified birth doulas since January 1, 2025 for one prenatal intake visit, continuous labor and delivery support, and up to 20 hours of other prenatal and postpartum visits. HCA's fee schedule allows $750 for the intake visit, a flat $750 for labor and delivery support, and $25 per 15-minute unit for other visits, which comes to a maximum of $3,500 per pregnancy by our arithmetic.
