What Bupropion does
Bupropion blocks reuptake of norepinephrine and dopamine rather than serotonin. That makes it distinct from SSRIs in two valuable ways: it does not cause sexual dysfunction, and it reduces cigarette cravings (branded as Zyban for smoking cessation). It is a reasonable first-line antidepressant and often a useful augmentation strategy.
What the evidence says for Black patients
Bupropion is an important option for Black patients for two reasons:
- Tobacco disparity. Black Americans smoke at slightly lower rates than white Americans but suffer disproportionately higher rates of smoking-related lung, esophageal, and oral cancers — driven in part by the menthol-cigarette marketing that the FDA has only begun to restrict. Bupropion doubles smoking-cessation success compared to placebo (Hurt et al., NEJM 1997;337:1195–202). A 2022 Cochrane review confirms efficacy across populations including Black smokers.
- Sexual side effects. SSRI-induced sexual dysfunction is a major under-reported reason for antidepressant discontinuation, and rates are particularly high (30–60 percent) for paroxetine and sertraline. Bupropion preserves sexual function; switching from an SSRI to bupropion or adding bupropion is a well-established rescue strategy (Clayton et al., J Clin Psychiatry 2004).
Bupropion is metabolized by CYP2B6 — an enzyme with variable activity across ancestries. CYP2B6 poor metabolizers may have higher bupropion levels; no specific dose adjustment is recommended, but awareness helps when side effects occur at low doses.
Bupropion lowers the seizure threshold and is contraindicated in patients with active seizure disorder, eating disorders (due to increased seizure risk with purging), and concurrent MAOI use.
Common alternatives
SSRIs (sertraline, escitalopram) for first-line depression. Varenicline (Chantix) for smoking cessation. Mirtazapine for sedating, appetite-restoring depression.

- Insomnia, activation
- Dry mouth
- Headache
- Nausea
- Seizure risk — dose-dependent (rare at XL ≤450 mg)
- Hypertension (usually mild)
- Anxiety
Factors that affect adherence
Take bupropion XL in the morning to avoid insomnia. Generic bupropion is inexpensive. For smoking cessation, combining bupropion with nicotine replacement improves quit rates beyond either alone.
