What Omeprazole does

Omeprazole blocks the gastric acid pump, dramatically reducing stomach acid. It's available over-the-counter for short courses of heartburn, and prescription for GERD, ulcers, and esophagitis.

What the evidence says for Black patients

No race-specific efficacy differences. But two equity-adjacent issues:

  • CYP2C19 metabolism. Omeprazole is activated by CYP2C19. Poor metabolizers (more common in some Asian populations; present but less common in Black populations) have higher omeprazole plasma levels and more acid suppression. Most patients don't need dose adjustment, but the clopidogrel interaction (omeprazole inhibits CYP2C19 and reduces clopidogrel activation) is documented — see the clopidogrel page.
  • Long-term PPI use. Associations with kidney disease, bone fracture, B12 deficiency, and C. difficile are reported. Given the disproportionate CKD and fracture burden in aging Black populations, deprescribing PPIs when no longer needed is a high-value intervention.

Common alternatives

Pantoprazole, esomeprazole — other PPIs with different pharmacokinetic profiles. H2-blockers (famotidine) for mild reflux. Lifestyle modifications (weight loss, elevation of head of bed, avoiding late meals).

Side effects

  • Headache, GI upset
  • B12 deficiency (long-term)
  • Magnesium deficiency
  • Increased risk of pneumonia, C. difficile
  • Bone fracture (long-term)
  • Small CKD risk (long-term)
  • Rebound acid hypersecretion on discontinuation

Factors that affect adherence

Take 30 minutes before breakfast. Generic OTC omeprazole is inexpensive. Re-evaluate need annually — chronic PPI use is often continued out of inertia.
