Well-Integrated Screening and Evaluation for Women Across the Nation
The CDC WISEWOMAN program (Well-Integrated Screening and Evaluation for Women Across the Nation) funds 21 states and 4 tribal organizations to deliver free blood-pressure, cholesterol, and diabetes-risk screenings to women 40-64, plus lifestyle coaching referral. WISEWOMAN partners with the NBCCEDP state programs — a woman signing up for a free mammogram is typically asked if she wants a free heart-health check too.
States without a WISEWOMAN grant run variations through their state health department's heart-disease prevention unit, but the free-screening pathway is state-specific and often region-limited.
Why this matters for Black families
Black women die from cardiovascular disease at roughly 1.3× the white-women rate (AHA, 2024 statistics update). Hypertension prevalence among Black women is 58% — the highest of any group tracked by the CDC.
The American Heart Association's 2024 statistical update places cardiovascular mortality among Black women at roughly 1.3× the white-women rate, with hypertension driving much of that gap. CDC BRFSS data show 58% of Black women ages 20+ have hypertension — the highest of any demographic group the CDC tracks.
WISEWOMAN's design centers this gap: the program targets women 40-64 (pre-Medicare age), offers onsite blood-pressure and lipid panel, and bundles the screening with lifestyle coaching through the same community clinics that run the state's NBCCEDP program. Every WISEWOMAN enrollee with elevated readings gets referred to primary care with an active navigation handoff.
Who qualifies (federal baseline)
Federal WISEWOMAN eligibility:
- Age: 40-64
- Income: up to 250% FPL (aligned with NBCCEDP)
- Sex/Gender: women (WISEWOMAN is women-only by statute)
- Insurance status: uninsured or under-insured
WISEWOMAN funds 21 states plus 4 tribal organizations as of the 2023-2028 cycle; other states run smaller heart-health outreach through their NBCCEDP grantee without the WISEWOMAN dollar. State-specific eligibility overrides follow the NBCCEDP-linked program.
