On maternal health specifically: HFS Medical Programs has extended postpartum Medicaid coverage to 12 months (effective 2021-12-01), and covers doula services through Medicaid at up to $1,450 per full perinatal package (effective 2024-04-01).

Illinois was the first state in the country to extend postpartum Medicaid coverage to 12 months (April 2021 under Section 1115 waiver, converted to a State Plan Amendment December 2021). Chicago Birthworks Collective and the Chicago Birthing Project serve Black pregnant people across Cook County.

Roughly 595,000 Black residents are enrolled in HFS Medical Programs, per the most recent CMS T-MSIS analytic file. The number understates true eligibility: every state has Black eligibles who aren't currently enrolled, usually because of the administrative-churn rates that rose sharply during the 2023 unwinding.
